AM, NH, and CA Sample Submission & Customer Information Forms
When submitting AM, NH, and CA samples, we require TWO forms be filled out and sent to us. You must complete both of the following forms, then submit it to Stockman's.
Step 1) Sample(s) Submission Form Fill out the following form and print it:
Step 2) Customer Information Form Fill out the following form and print it, mail it in along with the forms you filled out in Step 1. Failure to send both forms can result in your samples not getting processed:
